Fifth batch of ISO 639-3 modifications

Here is the fifth batch of ISO 639-3-based records and registration
forms. This is from the Addendum page at the end of the Summary of
Outcomes document, and consists of two new code elements for newly
identified languages. In other words, it is just like the third batch,
except that the 639-3 effective date was 2013-05-20 instead of
2013-01-23. I kept these separate to reflect the document organization
(plus the third batch was large enough already).
