Addendum to 2012 ISO 639-3 changes

I’m just about finished with a big project that took precedence over getting the 639-3-based subtags forms out. I hope to be able to get to those very shortly.

Doug, anyone, let me know if help is needed (I can probably handle up to ten changes). Or have these change requests already been made and am I somehow outdated? If the latter, my apologies. (I just don't see the change requests on the list.)
