ISO 639-3 changes, part 3

Per Kent's suggestion, here are revisions of those registration forms 
sent as part of the February 11 batch which make mention of old and new 
639-3 code elements with the same reference name.  The cited examples 
were "old Caló," which included Erromintxela by implication, and "new 
Caló," which doesn't.

These revisions simply add the word "proper" as indicated; Kent has 
indicated that this will resolve his concern.  The forthcoming Part 4 
registration forms will also take this into account.
