Clerical change on registration forms

I plan to edit the "Comments:" block at the end of each of Michael's
registration forms to start with "6. Any other relevant information:" as
required by BCP 47.

I don't plan to make any other changes that I might otherwise make,
including line wrapping to 72 characters (not actually required by BCP
47) or removing the empty Comments field in the "Record Requested"
blocks.

Please reply to the list before the end of the review period (January 11
AoE for everything) if you have any comments, or want me to post the
modified versions to the list.
