Here are the updated forms with this change (both forms), and with the
"Any other relevant information" change that I mentioned on January 4
(registration form only).

These are minor changes to an outstanding request, so they only need one
more week or review, and could be sent to IANA as early as Friday,
February 17. If we want to make IANA's life easier, we can wait another
6 days and send them this plus the 33 ISO changes at once.
