Cornu-English forms

The original request was posted on October 28, but with "Basic English" 
in one place (the email subject line doesn't really matter) instead of 
"Cornu-English". A proposed record also was not posted. Both are here 
for a one-week review.

Kent Karlsson suggested that these should say "Cornish English"; if that 
is desired instead of "Cornu-English", now would be the time to discuss 
that.
