Khotanese and Sakan

ISO 639-2 has announced a change to code element 'kho', adding the name 
"Sakan" to the existing name "Khotanese."

They've also added a note:

"Previously Sakan was in 639-3 as a separate code element (xsk), but it 
will be retired as a duplicate."

We should make the corresponding change to 'kho' in the current 
Registry, but we need to hope that 639-3 removes 'xsk' soon, as noted, 
or draft-4645bis is going to have a duplicate name.
