Final forms related to variant 'tarask'

By popular request, here are the final versions of the two forms to be 
sent to IANA in support of this subtag.  Please make any comments or 
corrections within 24 hours, by 0700 UTC Saturday.  I am fairly sure the 
list consensus was that we did not want this step to take another 2 
weeks.

First, Jaska's registration form, with the Comments field changed to 
match the final agreed-upon book reference.  Note that this reference is 
no longer the same as the one listed in item 4, "Intended meaning," but 
both books are listed in item 5.
