Q1 is mapping on lookup permanent or transitional?

It doesn't presume anything of the kind.  The fact is that if browsers
stop checking mapping on each lookup, then that part of the protocol
will just get less use.  You may be right that there's no way to tell
in advance what will happen there.  But in any case, the 2d lookup
remains part of the protocol, and therefore is always going to be
possible.
