ISO 639-3 adds Eskayan and Pattapu

ISO 639-3/RA has updated their list of changes in the 2013 request
cycle, adding two new code elements, [esy] for Eskayan and [ptq] for
Pattapu. There is also a revised Summary of Outcomes document.

Below are the records and registration forms to add corresponding
language subtags to the Registry. The expected submission date is April
5.
