ISO 639-3 changes: batch 5

This is the final batch of ISO-induced changes. These two changes were 
not directly mentioned in the Summary of Outcomes document, so they form 
a category of their own.

In both cases, the action taken by the RA is described in the PDF file 
mentioned in the registration form. Please note again that the RA's 
action, not the original user request, is the basis for changes to the 
Language Subtag Registry.

With the posting of all proposed changes today, the two-week review 
period will run through February 27 and the changes can be sent to IANA 
on the 28th. Any clerical modifications require only a one-week review, 
so they would not affect this date. Substantive changes, such as the one 
Kent Karlsson has proposed for 'sap', would restart the review period 
for just the affected subtag(s).
