IANA registration issues not covered by the "RFC 3066bis" draft

Registration is done on the basis of the IETF tradition of rough consensus
and running code. Michael does what we tell him to do (but "we" includes
Michael himself, indeed he is a highly regarded member of "we") in
accordance with that tradition.
